Documents to be Submitted along with the Filled COMEDK UGET 2012 Application Form :
COMEDK UGET 2012 Applicants must send their COMEDK UGET 2012 Application with the Necessary Documents on or Before 10th April 2012 to the Address Mentioned above
1. Demand Draft DD:
Applicants must submit their Demand Draft drawn in favour of COMEDK, payable at Bangalore. (DD must be payable at Bangalore only) for Rs.800/- (Eight Hundred Only) for 3 subjects (PCM or PCB) for MBBS/BDS or B.E and Rs.1000/- (One Thousand Only) for 4 subjects (PCMB) for MBBS/BDS and B.E.
COMEDK UGET 2012 Application Number, Applicants Name & Contact Number to be written at the back of the Demand Draft. Applicants must Check for the bankers signature on the Demand Draft before sending.
2. Printed COMEDK UGET 2012 Online Application form :
The Printed COMEDK UGET 2012 Application must have the
Applicants Photo with in the Box Provided (Should be Pasted not Stapled).
Signature, Left forefinger impression of the Applicants.
Signature of the Parents/Guardians.
3. Printed Online Date of Birth Certificate / Identity Proof (COMEDK copy) :
Attestation (within the space provided) by the Head of the Institution where he/she last studied or Gazetted officer not below the rank of Thasildar of the area in which he/she is a resident.
Resent Passport Size Photo and Signature of Applicant should be Pasted on the DOB and Identity Proof Certificate.
4. Caste Certificates for Reserved Categories (SC/ST/ OBC) Applicants.

All the Applicants who register and Filled COMEDK UGET 2012 Online Application Must Send their COMEDK UGET 2012 Application form On OR Before 10th April 2012 to the COMEDK Office Through Speed Post/Courier tot the Following Address.
